I, as the parent / guardian of, the above mentioned child (ren), is a minor, and I authorize and give consent to have the aforementioned child(ren) receive a haircut / hair styling at Craig Blac’s Community Cuts for Kids. I release and discharge Craig Blac / Craig Shields / The Craig Shields Foundation and all volunteers, sponsors, and affiliates associated with Craig Blac’s Community Cuts for Kids from all claims, liability, demands, lawsuits, damages, costs, and causes of action, known or unknown, fixed or contingent, which I may have or claim to have against Craig Blac / Craig Shields / The Craig Shields Foundation and all volunteers, sponsors, and affiliates of Craig Blac’s Community Cuts for Kids and further agree not to file a complaint or lawsuit to assert any such claims. This Release and Waiver includes but is not limited to claims pursuant to any Federal, State, or Local laws. I acknowledge I have completely read and fully understand all of the provisions of this Release and Waiver, and I have not relied on any representation, statement, written or oral, not set forth in this document.
